With CelebrityDDS dental practices can reward and encourage loyalty with memberships. Each practice has a unique membership package that allows existing patients like you to purchase services and dental products at membership only prices.
By joining the membership program you and your family can recieve the same high quality care for lower prices and free additional benefits. This is not an insurance plan, no claims are filed and no payments are made to any other healthcare provides. CelebrityDDS allows patients to schedule dental appointments and redeem offers that are provided by their dentist Membership. Once you and/or your family become Members by paying a small annual fee, you are entitled to your benefits for 12 consecutive months. Members, spouse, and dependent children under 21 years of age are eligible. Children may be members only as dependent S of adult members. You have the right to cancel at anytime within 30 days, however, this plan is non-refundable.

Eligibility:
Patient Must be an active patient of record of his/her dental clinic
Patient may enroll his/her spouse and eligible dependents.
Eligible dependents include your unmarried children to age 21.
